The CARE Form is now available for faculty and staff who would like to connect a student needing extra support to campus resources. The purpose of the CARE form is to identify students who may be facing one or more personal challenges. The form will be received by the Associate Dean for Case Management who will work collaboratively with the Class Deans and the Associate Deans within the areas of Student Life and the Maytum Center for Student Success to create a holistic plan of support for the student and connect them to resources.
