The Meadville Medical Center & Titusville Area Hospital are conducting this survey to identify the health needs in our community. Every resident of Crawford County is invited to participate.
The survey will NOT ask for your name, address or other identifiable information; however, you must reside in Crawford County (PA) in order to participate. If you are a student who lives in Crawford County for more than 6 months out of the year, you are eligible to participate (i.e. Allegheny Students). You do not have to answer any question that you do not feel comfortable answering. All information recorded through this survey will remain confidential.
